IMPORTANT: 

IF YOU ARE REGISTERING WITHIN 3 BUSINESS DAYS OF THE PROGRAM DATE, PLEASE MAKE SURE YOU HAVE ACCESS TO THE CHILD'S PHYSICAL (WITHIN 2 YEARS) AND IMMUNIZATION RECORD. THESE ARE REQUIRED FOR PARTICIPATION (State of Connecticut School Records are accepted). Make sure that you add each day of Camp MATT: School's OUT! for each registering child (there is a drop-down with any additional participants) before hitting 'Continue.'
